Pinnacle In-Home Care service is an independent, non-medical, in home care agency. We are licensed, insured, and bonded. Some of our services include: Personal Assistance, Alzheimer’s and Dementia care, meal preparation, bathing/personal hygiene, light housekeeping, transportation, and companionship. We also offer Case Management, Assessments for Medical Equipment, Transitional services, non-medical transportation referrals, and comprehensive Readmission Prevention Care for those who have recently discharged from a facility.
All our consultations and assessments are performed by Registered Nurses that are on our staff and attend our Case Conferences. Our clients are reassessed by one of our nurses approximately every 60 days to ensure the highest level of care. Each client receives a personalized care plan that ensures every requested detail is met! Our nurses provide ongoing communication between family, health care providers, and financial representatives. Our clients also receive education and coaching to families or clients on specific disease management.
Our personal assistants and caregivers are hand selected for each client based upon skills, personality, and the client’s specific needs. Each assistant has their background checked, driving record checked, is drug tested, and their TB test is current. Pinnacle In-Home care assistants are certified nursing assistants, in home care assistants, or personal care assistants.
